Suggested Nursing Care Plan for Patients with End Stage Kidney Disase

End Stage Kidney Disease is not an independent disease, but a common clinical syndrome of various kidney diseases developing into the late stage. Then, for uremia patients, how to nursing care in daily life?

1. When breathing has the smell of ammonia, the patients easily suffer from stomatitis, which should strengthen the oral care;
2. Because of a brain abnormality or low calcium, the patients may show convulsion, at this time, the family should protect patients from self-injury, and immediately notify the physician;
3. The patients with frequent vomiting and diarrhea should pay attention to water and electrolyte disorder; when the relevant symptoms appear, the family should promptly notify the physician.
General nursing:
1. Skin care: Because of metabolites retention causing skin itch, the patients can wash with hot water, do not scratch the skin with the hand in order to prevent the infection;
2. The patients on bed rest having unconsciousness, dysphoria, convulsions and coma should be put bed bar and strengthened patrol to prevent the patients falling from the bed;

3. The patients should keep high calorie, vitamin and high quality low protein diet; the patients can regulate the protein intake according to their renal function; the patients with high blood pressure should limit the sodium intake; and the dialysis patients should be given high quality protein.
